At Lead the Way Support Coordination, we offer a compassionate and non-judgmental approach that empowers individuals living with disabilities to reach their goals.

In your role as a Specialist Support Coordinator at Lead the Way Support Coordination, your responsibilities include assisting a caseload of NDIS participants in effectively implementing all aspects of their NDIS plans. This includes informal, mainstream, and community supports, as well as funded support services.

The position entails helping each participant enhance their capacity to independently coordinate their supports, with a strong emphasis on a "do with, not for" approach. You will offer expert NDIS guidance, support participants in building their self-reliance. As part of compliance with reporting obligations, you will provide the NDIA with reports detailing outcomes and success indicators in accordance with the agreed reporting time frames. Conducting home visits to participants is an essential aspect of this position.

Applicant are required to Live in South Australia, home visits with clients are required despite this being a fully flexible work from home role.

Tasks & responsibilities

Work collaboratively with multiple stakeholders and seek to achieve well-coordinated plan implementation.

Ability to coordinate and lead a complex multidisciplinary team of stakeholders for targeted interventions to achieve positive outcomes for the participant.

Facilitate plan implementation to utilise all support categories within the plan to ensure the participants can achieve their goals and maximise outcomes outlined in individual plan.

Liaise with NDIA and NDIS Quality and Safeguards Commission regarding mandatory reporting within legislative timelines to ensure safeguarding of participants as required.

Ensure that all participant files are up to date, support plans and annual reporting is complete and are of a high standard.

Work alongside the participant to identify gaps in service provision and link participants to a suitable provider ensuring that all needs identified are met to achieve positive outcomes and mitigate barriers to plan implementation.

Qualifications & experience

Experience working as a Support Coordinator or within the NDIS space as a Social Worker.

Significant experience working directly with participants with a disability or who have psychosocial disability.

Proficiency in using Microsoft Office programs and the ability to quickly adapt to and learn new software.

The ability and skillset to work from a trauma-informed, strength-based approach to attain positive client outcomes and achieve client goals.

Completion of a Certificate IV in Mental Health or an equivalent qualification.

Desirable - Completed the DSC Support Coordinator Training

Desirable - Bachelor of Psychology or undertaking currently.

Desirable - Bachelor of Social Work or undertaking currently.

Desirable - Knowledge of the Disability Services Act, National Disability Insurance Scheme Act, National Disability Services Operational Guidelines and the NDIS Operational Guidelines.

Salary:

This is a contracting role with two pay rates:

Support Coordination: $60 per hour

Specialist Support Coordination: $100 per hour

All superannuation and taxation is the responsibility of the contractor.
