A Role with Impact:
As a Specialist Support Coordinator, you will take on advanced responsibilities in providing tailored, high-quality support coordination services within the NDIS field. You’ll work directly with participants and their stakeholders to help implement their individual NDIS plans and also work within complex multi-disciplinary teams. This part-time role offers a dynamic blend of in-office collaboration, on the road support and the flexibility of hybrid work arrangements.
About the Role:
- Position: Part-Time Specialist Support Coordinator
- Start: Mid-Late March
- Hours: 22.8 hours per week - with potential to increase with ongoing referrals
- Award Level: SCHADS Level 5
- Location: Based in Inglewood with the opportunity for hybrid work from home
- Technology Provided: Laptop and phone included
- Benefits: Superannuation included
Key Responsibilities:
- Deliver high-quality psychosocial support coordination services to NDIS participants.
- Work with complex mental health and psychosocial issues.
- Assist participants in navigating the NDIS system and connecting with appropriate services.
- Develop and implement individualised plans that reflect participants' goals, needs, and aspirations.
- Prepare detailed, professional reports documenting participant progress, challenges, and outcomes.
- Perform essential administrative tasks, including updating records, managing schedules, and ensuring compliance documentation is maintained.
- Build strong relationships with participants, service providers, and community partners.
- Attend regular in-person meetings with your supervisor for guidance and support.
- Assume additional higher duties as required, including taking on leadership responsibilities, mentoring peers, and managing more complex cases to support continuous service excellence.
Selection Criteria:
The successful candidate will have:
- Significant experience in Support Coordination/Specialist Support Coordination working with psychosocial disability within the NDIS field is essential.
- A Certificate IV in Mental Health, or a Bachelor in Psychology, Social Work, Counselling or a similar field is required.
- Excellent interpersonal skills with the ability to engage effectively with participants, service providers, and various stakeholders.
- A strong knowledge and understanding of the National Disability Insurance Scheme (NDIS) and NDIS Practice Standards.
- Experience in case management and coordinating individual plans using community-based supports.
- Proven ability to manage complex situations with minimal supervision and effectively resolve issues.
- Strong written and verbal communication skills, including proficiency in report writing.
- Exceptional organisational skills with the ability to manage competing priorities.
- A current NDIS Worker Screening Check (or willingness to obtain one)
- Current Australian Drivers Licence and reliable car.
