Sage Support Coordination has an opportunity for a full time Support Coordinator or Specialist Support Coordinator to join our team of Support Coordinators based in Torquay on the Surf Coast this is for working with complex medical and physical disabilities such as hospital discharges.

Support Coordinators with Sage are provided with a mobile phone and laptop to enable them to be able to work effectively from the office, home, or with a participant.

We are looking for someone who meets the below requirements.

• an outstanding commitment to people,
• sound knowledge of the NDIS,
• previous experience as a Support Coordinator, min 6 year
• efficient computer skills
• enjoys working in a fast-paced setting.
• .8 considered for the role to full time.
