Family Support Navigator - Lead Service Coordination Across Regions

We are seeking a Family Support Navigator to play a key role in coordinating service pathways and ensuring families receive the right support at the right time. About the Role

As the Family Support Navigator, you will:

  • Coordinate referrals and service capacity across Meli’s Family Support Platform to ensure a streamlined and client-centred support journey.
  • Strengthen partnerships with local service providers to improve referral pathways and community responsiveness.
  • Advocate for families by ensuring they receive the most appropriate interventions based on assessed need and risk.
  • Work collaboratively across all family support service streams—connecting, strengthening, and restoring—to ensure an agile and integrated response.
  • Monitor data and service trends to identify gaps, improve efficiencies, and inform strategic service development.
  • Travel across Geelong, Central Highlands, and the Southwest to collaborate with internal and external stakeholders.

What We're Looking For

We value individuals who bring leadership, collaboration, and sector expertise to their roles. To succeed in this position, you will have:

  • A tertiary qualification in Social Work, Psychology, Human Services, or a related field.
  • A strong understanding of child and family vulnerability, sector trends, and the Victorian family services system.
  • Exceptional stakeholder engagement skills to build and maintain collaborative partnerships within a complex service environment.
  • Highly developed communication skills, with the ability to convey ideas effectively in both written and verbal formats.
  • Demonstrated ability to analyze data and feedback to improve service processes and ensure sustainable service delivery.
