About the Organisation
Care to Care is an approved HCP and DVA provider, and also a registered NDIS provider. We are delivering in-home assistance to Aged and Disabled clients across Brisbane and pride ourselves on our high quality and personalised service delivery.
You will be part of a very supportive team, led by passionate and involved owners. From a clinical perspective, you will be part of a great team and supported by a strong Care Manager (RN) who provides guidance and ongoing development.
Our main office is based in Cannon Hill; you would be based here and travelling to see your clients from here using our company vehicles.
Main Responsibilities:
- Client liaison and new Client onboarding
- Client Assessments, Care Plan development & Case Management
- Delivery of Nursing Services
- Support Worker matching and scheduling of services
- Direct and indirect delivery of clinical care (within scope of practice), ie medication management, wound management
- Working closely with fellow Care Coordinators and Support Workers
- On-call is shared across the team, on a rotational roster
- Ongoing Continuous Improvement & Risk Management
- Compliance, Incidents and Feedback Management
- Administrative activities, using the Care Management System
Key requirements:
- Minimum of 3 years’ experience as EN/RN, with current AHPRA Registration
- Experience as a Care Coordinator, with detailed understanding of different funding programs (HCP, NDIS, etc.) would be advantageous
- Outstanding interpersonal and communication skills
- A good listener with a positive, warm and & friendly personality
- Must be a team player, supportive, personable and reliable
- Calm natured, high on emotional intelligence and mentally resilient
- Excellent organisational and time management skills, with high attention to detail
- Professionalism, well presented, with a good reputation in the market
- Respect for our staff, clients and their families
- Advanced computer skills
- Full drivers’ licence
- Registered and insured vehicle available for work purposes
- Smart Phone & data
- Current First Aid & CPR
- National Police Check, Blue card and NDIS Workers Screening Check or willing to obtain
We offer a competitive remuneration package, in line with qualifications and experience under the relevant award with an additional above-award premium.
