About Us
South West Community Care is a not-for-profit Organisation and Registered Charity local to the South West, providing aged care and in-home services to the local community. It is managed by a Chief Executive Officer (CEO) who is supported by a Chief Governance Officer and Management Team overseeing operations and supporting a team of over 120 employees and volunteers. The CEO, in conjunction with a volunteer Board of Management, sets the strategic direction for the organisation.
South West Community Care is managed locally and embraces the ‘Wellness’ Approach when delivering care to eligible older Australians in the Community approximately 1000 clients currently, providing services designed to maintain independence dignity and quality of life for our ageing community. These services are delivered by compassionate and caring staff working within the guidelines set out by the Aged Care Charter of Rights and Code of Conduct for Aged Care to deliver person centred care to optimise positive outcomes.

Position Purpose
Reporting to the Chief Operations Officer, the purpose of your role is:
To provide clinical leadership to the care team, with a key focus on care planning, quality assurance and continuous improvement, maintaining accreditation standards and overseeing the provision of all lines of service to clients. To oversee and lead the delivery of high-quality person-centred care in the community setting which aligns with the Aged Care Quality Standards.
Position Responsibilities
As a Clinical Care Manager you will be responsible for and assisting with:
Operations
- Effectively lead a team of Care Partners and Care Coordinators in the management of services.
- Review intake of clients, ensure adequate care planning is provided and risk assessments are completed for all clients (CHSP and S@H).
- Review or undertake initial assessment of clients to ensure clinical assessments are completed at a high level to make way for effective delivery of services.
- Working with scheduling to ensure services are prioritised for high care clients between clinical, in home and contracted services.
- Ensure that all care aligns with current best practice as set out by Department of Health and Aged Care (DoHAC) and Australian Health Practitioner Regulation Agency (AHPRA).
- Contribute to clinical guidance to all levels within SWCC to ensure clinical governance.
- Act as an advocate for clients when it comes to liaising with appropriate parties such as DoHAC, GPs, specialists, Allied Health, and discharge planning coordinators, health care facilities and supportive service providers.
- Escalate situations which require review with recommendations as an Allied Health Professional to medical specialists and the MAC Portal.
- Monitor clients to identify deterioration or need for intervention.
- Mentor a Care Team to effectively set goals and care plans that are holistic and achieve goals set.
- Assist with family meetings, conflict resolution and clinical compliance as a health professional.
- Maintain quality indicators with Quality and Client Experience Manager.
- Provide coverage for Care Partners and Care Coordinators when required.
- Contribute to continuous improvement particularly when in regards to the implementation of all clinical care which under the Home Care Services is all services provided by SWCC.
- Work collaboratively with other departments and other members of the leadership team to ensure consistent service delivery.
- Support Care Team to adhere to the financial controls around purchasing and budgets.
- Review and approve purchase orders and billing to S@H clients.
- Implement additional means of providing rationale to all package funding and ensuring compliance.
- Apply for any additional supplements that clients have available to them.
- Review financial reports on performance of client budgeted services and contribute to service improvement and compliance and work with Care Team to identify target areas.
- Mentor Care Partners in adequately and creatively allocating budgets to maximise client outcomes.
To thrive in this role you will have:
- Demonstrated high level of clinical expertise in chronic disease management and documentation.
- Proven track record in understanding and meeting the requirements of accreditation within an Aged Care/Health environment.
- Sound clinical skills and knowledge of contemporary aged care practices including the use of software systems.
- Knowledge and understanding of the Aged Care Quality Standards.
- Demonstrated high-level communication and interpersonal skills, with the ability to build and maintain effective working relationships with employees, clients and stakeholders.
Office Base Location
Based at the Australind Office but will be required to work from other sites.
