Shine Support Coordination & Consultancy Services will work in partnership with Individuals, their families, mainstream providers and the disability sector to ensure a participant’s services are innovative, flexible and person-centred.

The Specialist Support Coordinator is responsible for implementing and coordinating individualised supports and services for Individuals with NDIS plans. In this role you will be responsible for providing specialist support coordination to Individuals with complex needs, behaviours of concern, medical needs, or other high-risk circumstances, to resolve points of crisis, assist in risk management and promote informed choice and decision making for Participants connected to the NDIS

MAIN DUTIES/RESPONSIBILITIES

Program Implementation and Development Expected Deliverables

Manage and coordinate services in participants’ NDIS plans including the management of complex multidisciplinary teams.

Assist the Individual at the specialist level to implement their NDIS plan by maximizing choice and control and engaging high quality service providers.

Develop action plans following consultation with Individuals, their families and other stakeholders to resolve challenges arising from complexity.

Ensure that the optimal mainstream and community supports are utilised by the Individual wherever possible and ensure the Individual receives value for money services when NDIS funds are spent to achieve the objectives of the Individuals plan.

Provide advice and consultation with Individuals, their families and other stakeholders in the language or mode of their choice, in response to complex and challenging situations.

Provide support and advice to other support coordinators in relation to support coordination delivery.

Manage and monitor risk.

Provide written and/or verbal reports as requested by the Chief Executive Officer.

Relationship Management Expected Deliverables

Develop and maintain effective communication and collaboration with workers of Shine SCCS.

Represent Shine SCCS on committees, working parties, forums, seminars and attend meetings, whilst acting in the best interests of the organisation.

Projects Expected Deliverables

Participate in internal and external audits aligned with the NDIS Quality and Safeguarding Commission Practice Standards to promote a culture of continuous quality improvement.

Collate feedback and evaluation results to embed the delivery of flexible, individualised and contemporary services for people with a disability.

Other duties as required.

SELECTION CRITERIA

Essential

Tertiary degree in Social Work, Psychology, Occupational Therapy or Allied Health or equivalent discipline with relevant field experience as per the NDIS quality and safeguarding framework requirement.

Demonstrated experience in complex case management.

Knowledge of National Disability Insurance Scheme and relevant legislation including the NDIS Act 2013 and Disability Act 2006.

Expert communication (both written and oral) skills and able to manage complex relationships.

Flexible and able to work effectively within a changing environment.

Demonstrated commitment to promoting a cooperative and cohesive team.

High level of computer proficiency in MS Office suite.

Current membership to qualification relevant industry peak body/associaton/Australian Health Practitioner Regulation Agency (Ahpra).

Desirable

1. Demonstrated experience in specialist support coordination.

2. Experience using FlowLogic software.
