The Role

We are looking for an energetic, compassionate professional who loves bringing people together and creating clarity in complex situations.

You’ll combine the role of NDIS (Specialist) Support Coordinator with that of Care Coordinator for our Support at Home (SaH) program for aged care, serving clients across the Perth Metro area.

Your day-to-day may include

• Helping clients understand and make the most of their NDIS or SaH plan

• Coordinating and planning care and support that aligns with each client’s unique goals

• Building warm, person-centred relationships based on trust and respect

• Setting up service agreements and bookings

• Working closely with clients, families, and care teams to promote choice and control

• Liaise and negotiate with other providers and organisations (B-to-B) to ensure clients receive the services that best meet their needs.

• Encouraging independence and supporting people to live life their way.

Requirements

• Minimum 2 years’ experience as a Support Coordinator (NDIS) or Care Coordinator (SaH, aged care)

• Australian Driver’s Licence

• NDIS Worker Screening Check

• Australian working rights
