The Role

As a Pharmacist in Aged Care at Priceline Pharmacy Fairfield, you will play a vital role in delivering comprehensive pharmaceutical care to residents in aged care facilities within the local area. This part-time position offers the opportunity to work closely with multidisciplinary healthcare teams to ensure optimal medication management and the overall wellbeing of the elderly population.

Key Responsibilities

Conduct medication reviews and provide recommendations to improve medication management for aged care residents

Collaborate with healthcare professionals, such as nurses and doctors, to develop and implement individualised medication treatment plans

Provide medication education and counselling to residents, their families, and aged care staff

Monitor medication safety and efficacy, and address any medication-related issues or concerns

Ensure compliance with relevant pharmacy regulations and guidelines in the aged care setting

Participate in continuous professional development to maintain and enhance your pharmaceutical knowledge and skills

What We're Looking For

Registration as a Pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA)

Minimum 2 years' experience working as a Pharmacist, preferably with exposure to the aged care sector

Strong clinical knowledge and expertise in geriatric pharmacotherapy

Excellent communication and interpersonal skills to effectively collaborate with healthcare teams and educate patients

Proficient in medication management software and information systems

Commitment to providing high-quality, patient-centred care
