Circle of Hope Services (COHS Pty Ltd) is an aged care registered provider delivering home care supports and services to people aged 65 and over who are eligible for government funding under the My Aged Care - Support at Home program (formerly known as Home Care Package program (HCP).
We Are Seeking
An experienced, passionate and dedicated Care Manager to join our Circle of Hope Services team. About the role
The role is predominately an office-based role, Monday to Friday 9:00 am to 5:00 pm based in Greenacre, South-west Sydney and will manage a caseload of participants as part of COHS's care planning, assessment and review team, the aged care portal and daily operations of the business.
Part of the role includes networking and building customer relationships, intake, care planning, assessment and review of consumer individual care plans, consumer risk assessments, medication plans and checklists, end of life plans, monthly budgets and rostering of care supports and services in the participant's home.
The successful candidate must have previous experience in aged care and administration, coordinating aged care services and solid knowledge and understanding of Aged Care legislation, regulatory requirements and the Aged Care Quality Safeguards Framework.
The successful candidate will also comply with COHS's values, code of conduct, policies and procedures and relevant government legislation, standards, rules and guidelines where relevant while maintaining the privacy and confidentiality of consumers, carers, families and the organisation.
Selection Criteria
Tertiary qualification and significant experience working in the Health and Aged Care services sector (Aged Care, Disability, Nursing, Mental Health, Social Work, or Business Management).
A minimum of two (2) years' administration and case management experience and solid knowledge and understanding of the Aged Care legislative framework and regulatory requirements (Aged Care Strengthened Quality Standards).
Demonstrated ability to be person-centered focused and work collaboratively with participants, registered supporters, carers, loved ones and stakeholders.
Results driven with the ability to self-motivate, multi-task and achieve results within a Key Performance Indicator framework.
Strong administration, written and computer skills for the creation of reports, correspondence and other documents in a cloud-based client and records management system.
Follow COHS's records management policies and procedures.
Strong interpersonal skills with a positive attitude and a flexible and adaptive nature to respond to the changing needs of our participants.
The successful candidate will be subject to completing the Aged Care ALIS eLearning modules as part of the induction process.
Current NSW Driver's License and own vehicle.
National Police Check (valid within the last 90 days)
Right to work within Australia.
