About the opportunity
IFYS currently has a newly created opportunity for a Service Coordinator – Wide Bay to join our Advance to Zero Homelessness initiative in the Wide Bay region.
This role is responsible for convening and facilitating multi-agency service coordination meetings, supporting individuals and families on the By-Name List (BNL) through housing pathways and wrap-around services. The Service Coordinator will work closely with health and housing sectors to support the shared mission of creating a community where homelessness is rare, brief, and non-recurring.

- Contract until 30th June 2026, 60.8 hours per fortnight

- Salary base $88,000 per annum + superannuation – IFYS is committed to gender pay equity

Key responsibilities
- Facilitate multi-agency service coordination meetings and community engagement workshops that are aligned with Project Zero methodology and responsive to sector needs
- Track actions and outcomes for individuals and families on the BNL using CSnet and Excel
- Collaborate with Wide Bay Hospital and Health Services to align housing and health pathways
- Strengthen partnerships with councils, domestic violence services, youth providers, and other stakeholders
- In collaboration with the Project Lead, support frontline staff to apply the Australian Homelessness Vulnerability Triage Tool (AHVTT) and Project Zero methodology
- Promote campaign membership and engagement to create service directories, prioritise memberships based on system gaps for people experiencing homelessness
- Provide detailed monthly updates, and feedback to the Zero Steering Group and contribute information for stakeholder reporting
To be successful in this role you will have
- Demonstrated professional experience in housing, homelessness, or community services sectors
- Strong facilitation, coordination, and stakeholder engagement skills
- Proficiency in data tracking and reporting tools (Excel, CSnet)
- Understanding of Housing First principles and Zero methodology
- Excellent interpersonal, written, and verbal communication skills
- A Certificate IV or higher qualification in a relevant area is highly regarded
Must hold or be willing to obtain if successful
- Valid Queensland open driver's license with no restrictions, and own reliable transport; clear traffic record
- QLD Working with Children Check (Paid Blue Card)
- Licensed Care Suitability Check (LCS2)
- National Criminal History Check (a new check will be conducted through IFYS systems)
- Ability to provide two recent line manager references including your current employer
