About the Opportunity
As a Senior Care Partner (Case Manager), you will play a key leadership role within the Aged Care team, overseeing Care Partners and Support Workers to ensure the delivery of high-quality, culturally safe, and consumer-focused care. You will be responsible for developing and reviewing care plans that support older people to live independently at home, aligning services with their goals, preferences, and budgets.
The role involves coordinating services across programs such as Support at Home, CHSP, TCP, and fee-for-service work, while maintaining accurate records, managing budgets, and promoting services within the community. You will also provide supervision, mentoring, and support to staff, contribute to continuous improvement initiatives, and participate in out-of-hours work and the on-call roster as required.
What You’ll Bring:
- Qualifications in Nursing, Allied Health, Social Work, or Community Services.
- Experience in aged care service delivery, care planning, and assessments.
- Strong interpersonal skills and cultural competency, especially with Aboriginal and Torres Strait Islander communities.
- Knowledge of aged care reforms, compliance, and the My Aged Care portal.
- Prior experience or, at a minimum, demonstrated commitment to working respectfully with Indigenous and culturally and linguistically diverse clients, communities, staff, and Aboriginal Controlled Organisations

Please note: It is a requirement that prior to commencing in this role, you must provide 100 points of identification to confirm Australian citizenship or eligibility to work in Australia; possess a Working with Children (Ochre Card), a current National Criminal History Check (within 3 months) and a valid NT driver’s licence.
Experience of working closely with and having a strong understanding of Aboriginal & Torres Strait Islander culture would be considered an advantage.
