About the role

Join the team at Plan Wise Living as a Support Coordinator (NDIS) in Oran Park, NSW. In this part-time role (with the option to increase to full-time), you will play a crucial part in working with individuals with disabilities to access the services and support they need to live independently and achieve their goals.

What you'll be doing

Develop and implement individualised support plans to meet the unique needs of NDIS participants

Coordinate the provision of services from a range of providers, ensuring seamless delivery and high-quality care

Advocate on behalf of participants to secure appropriate funding and resources from the NDIS

Build and maintain strong relationships with participants, their families, and a network of service providers

Monitor and review participants' progress, adjusting support plans as needed

Provide guidance and assistance to help participants navigate the NDIS system

Maintain detailed records and documentation in line with NDIS and organisational requirements

What we're looking for

Minimum 2 years' experience in a similar support coordination or case management role, preferably within the disability or aged care sector

Demonstrated understanding of the NDIS and its participant-centred approach

Understanding of the role of Public Trustee and Guardian (TAG), developing and maintaining a strong rapport

Understand and work within the Mental health sectors, including Health, Justice and Correction Services

Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse stakeholders

Strong problem-solving and decision-making skills, with a focus on achieving positive outcomes for participants

Proficient in using technology and relevant software to maintain accurate records and reporting

Preferred (but not essential) Relevant tertiary qualification in a human services or allied health discipline (e.g., social work, occupational therapy, psychology)

Willingness to travel to meet with participants in their homes or community settings
