About the Opportunity

As a Care Coordinator, you will play a key role in planning and delivering high-quality, consumer-focused aged care services. You’ll work collaboratively with consumers, families, carers, and support staff to develop and review care plans that enhance wellbeing and meet individual goals within budget.
You’ll support programs including Home Care Packages (HCP), Commonwealth Home Support Program (CHSP), Transition to Care Program (TCP) and fee-for-service work. This role includes occasional out-of-hours work and participation in the on-call roster
What We’re Looking For:
- Qualifications in Nursing, Allied Health, Social Work, or Community Services.
- Experience in aged care coordination and care planning.
- Knowledge of aged care reforms, compliance, and the My Aged Care portal.
- Solid administration, ICT, and financial management skills.
- Prior experience or, at a minimum, demonstrated commitment to working respectfully with Indigenous and culturally and linguistically diverse clients, communities, staff, and Aboriginal Controlled Organisations.

Please note: It is a requirement that prior to commencing in this role, you must provide 100 points of identification to confirm Australian citizenship or eligibility to work in Australia; possess a Working with Children (Ochre Card), a current National Criminal History Check (within 3 months) and a valid NT driver’s licence.
Experience of working closely with and having a strong understanding of Aboriginal & Torres Strait Islander culture would be considered an advantage.
