About Veritable
Veritable is a trusted NDIS-registered provider committed to supporting individuals and families to live more independently and confidently. We focus on person-centred care, dignity, and community inclusion across our range of social and disability support services.
About the Role
We are looking for a qualified and compassionate Social Worker to join our Alice Springs team. In this role, you'll work closely with individuals, families, and service providers to assess needs, develop care plans, and support clients with the right services to meet their needs.
Why Join Veritable
Work with a supportive and purpose-driven organisation.
Make a real difference in the lives of people in the Alice Springs community.
Enjoy ongoing learning and professional development opportunities.
If you're a dedicated Social Worker who values integrity, care, and empowerment, we'd love to hear from you.
Position Description
You will work closely with individuals, families, service providers and community organisations to assess needs, conduct individual and carer interviews, review data, develop specialist support plans, and ensure clients have appropriate services. You will play a key role in improving client wellbeing, promoting human rights, and building more inclusive communities.
As a social worker, you will provide social work interventions for young people and adults with disability (primarily NDIS participants). Interventions will be delivered within a positive behaviour support framework, with a social work focus on assessment, casework, advocacy, training and connection to community.
You will work with clients who may have complex needs, including contact with the justice or acute mental health systems, homelessness, anxiety or situational crisis. Many clients are from culturally and linguistically diverse backgrounds and some may be under adult guardianship.
This is a full-time role based in Alice Springs, Northern Territory. You will work locally and require the skills to establish rapport and make connections with families and support services, including cross-culturally and in remote Aboriginal communities.
Short trips to remote communities may be requested for specific referrals or clients on the caseload. Supervision and support will be provided to develop practice-specific skills and knowledge relevant to the position. You will compile case notes and reports, monitor progress, and collaborate with multidisciplinary teams to improve client outcomes.
Remote community travel is expected infrequently, typically involving only a few trips per year for specific client needs.
Key Responsibilities
Assess clients' social, emotional and practical needs through individual, service and family case interviews, and identify priorities for intervention using a structured practice framework.
Develop and review personalised support and intervention plans with clear goals, timeframes and outcome measures.
Collaborate on plans with clients, families, carers and allied health professionals, and provide guidance and training to support implementation.
Where required, support clients to link to mainstream resources and services and act as a facilitator between clients and services.
Provide counselling, advocacy and mediation within scope of practice and organisational needs, while supporting client rights, safety and participation.
Collaborate with the client, service providers and relevant stakeholders to improve service coordination and address service gaps.
Conduct risk and safety assessments, escalate concerns and implement agreed risk management actions.
Monitor client progress, maintain accurate case notes and compile reports consistent with organisational and regulatory requirements.
Use qualitative and quantitative data, research and observation to evaluate outcomes and adjust interventions.
Identify any use of restrictive practices in client environments and work proactively with stakeholders to reduce and cease such practices in line with policy and best practice.
Contribute, when directed, to community development activities and to the analysis and improvement of local social policies and procedures relevant to disability and inclusion.
