The Role
My DSS is looking for a Support Coordinator Assistant (Junior) to join our Campbelltown team on a permanent full-time basis. The purpose of this role is to work under the Senior Support Coordinator in the company to assist NDIS participants in the coordination of their supports funded in the NDIS plans. It involves doing the admin work needed by management in monitoring and implementation of all supports in their funded plan, as well as funded supports. The goal is to ensure that participants can achieve their expected goals including of independence and well-being so that they are better able to participate in community. You'll be working as part of a passionate, supportive and flexible team who values collaboration and career progression into becoming a Support Coordinator.
Main responsibilities include:
Doing admin work to Coordinate the day-to-day operation of the NDIS Support Coordination Services
Contribute towards participant assessment care planning, response to referrals and reviews in line with MDSS'S Policies and procedures.
In collaboration develop and promote a team-based approach to deliver participant centered care and services
Collaborate with participants, fostering an understanding of their needs and aspirations to explore options and develop actions that best meets their care needs.
Gathering data, booking appointments, supports and services
What you need
Minimum 6 months to 1 year experience as a Disability Support Coordinator - knowing the basics.
Qualification or enrolled in a course for Disability Services, Aged Care or other relevant Community Services
Experience working with NDIS participants with knowledge of NDIS Operational Guidelines
Highly developed communication and interpersonal skills
Team player who is driven by compassion and role purpose
An Australian driver's license, Working with Children Check, National Police Clearance, First Aid Certificate and a NDIS Workers Screening Check
Own vehicle and comprehensive car insurance
Excellent admin, office and PC skills.
