Your Role will be to:

Provide care to support and maintain a client’s independence, safety, and wellbeing

Be well organised, confident, and able to cope in potentially challenging situations

Support and guide community care providers in the safe and effective delivery of care

Your Key Responsibilities will include:

Monitoring chronic conditions and assessing the need to provide client specific nursing care and advice.

Collaborating with RNs, General Practitioners, Specialists, Allied Health Professionals, Practice Nurses and families to optimise client outcomes

Following individualised nursing care and wound care plans for clients who require nursing intervention. (Wound care experience is desirable)

Collaborating closely with Home Care Partners to provide client and family support with a wholistic approach

Communicating confidently both verbally and in written correspondence and documentation.

Providing wound care and management plans for a wide range of wound types under the guidance of our RNs

Use of Infection Control and Prevention best practice recommendations in all clinical interactions

Care and monitoring of urinary catheters and stomas.

Strong problem solving and analytical skills for delivering safe nursing care.

Delivering education to support staff at regular intervals under the guidance of the Clinical Coordinator.

Attend fortnightly team meetings and contribute to continuous improvement

Uphold all AHPRA requirements and ethical standards
