Registered Nurse (Community – Home Care Provider)

Position Overview

The Registered Nurse (RN) plays a vital role in delivering high-quality clinical care and support to clients living independently in the community. Working within a multidisciplinary team, the RN ensures that all care provided aligns with best practice, clinical governance, and the organisation’s values. This role focuses on promoting independence, dignity, and wellbeing for clients through assessment, planning, and delivery of person-centred nursing care.

Key Responsibilities

Clinical Care and Coordination

Conduct comprehensive clinical assessments and develop, implement, and review individualised care plans.

Provide skilled wound care management, including assessment, dressing selection, and monitoring of healing outcomes.

Administer and monitor end-of-life medications in accordance with palliative care protocols and client care goals.

Undertake continence assessments, recommend appropriate products and strategies, and support continence management plans.

Ensure all care is delivered in line with current evidence-based nursing practices, organisational policies, and regulatory requirements.

Education and Leadership

Provide education, supervision, and mentorship to support workers and enrolled nurses to promote safe and competent care delivery.

Deliver training in clinical procedures, infection control practices, and medication management as required.

Support the professional development and competency of team members through ongoing feedback and guidance.

Clinical Governance and Risk Management

Lead and participate in incident management, including reporting, investigation, and follow-up actions to improve client safety.

Oversee infection prevention and control practices across service delivery to minimise clinical risk.

Participate in clinical management and case review meetings, contributing to quality improvement initiatives and policy development.

Maintain accurate and timely clinical documentation and ensure compliance with all regulatory and accreditation standards.

Communication and Collaboration

Collaborate with GPs, allied health professionals, families, and other stakeholders to ensure coordinated and holistic care.

Advocate for clients’ rights, preferences, and needs in all aspects of care.

Communicate effectively within the multidisciplinary team and contribute to a supportive and positive work environment.

Qualifications and Requirements

Current AHPRA registration as a Registered Nurse.

Minimum of 2 years’ post-graduate experience (community or aged care preferred).

Demonstrated experience in wound care, continence management, and palliative care.

Strong knowledge of infection control, clinical governance, and risk management frameworks.

Current CPR certificate, First Aid, National Police Check, and Working with Vulnerable People/Children Check (as required).

Current Driver’s Licence and access to a reliable vehicle.
