Health N Home is seeking experienced and reliable Support Workers to provide consistent daily support to an NDIS participant in the Jimboomba / Browns Plains / Beaudesert region.

About the Role

Support is required 6 hours per day, every day of the week.
Shifts can be structured as:

2 × 3-hour shifts (AM & PM), or

3 × 2-hour shifts (AM, Midday & PM)

Duties include:

Domestic assistance throughout the day

Showering and dressing in the morning

IDC support — empty/remove overnight drainage bag in AM and reconnect before bed

Meal preparation as requested

Approx. 14 hours of community access per fortnight (appointments & social support)

Full escalation support from Health N Home clinical team

Two workers will be engaged to share the roster for continuity and reliability.

Driver’s licence & insured vehicle

Qualifications

Certificate III in Individual Support (Ageing or Disability), or

Certificate III in Aged Care / Community Services, or

Demonstrable equivalent experience in similar role
