The Butler Canberra is looking for an experienced and passionate Care Manager (ideally with qualifications in Allied health / Nurse / Physiotherapist etc but not necessary) to join our team and lead the delivery of high-quality, person-centred home care services. This is a pivotal role for someone who thrives in the community setting and is ready to support both our clients and our organisation through the changing aged care landscape.
As the sector moves towards full implementation of the new Aged Care Act, this role offers the opportunity to be at the forefront of clinical compliance and care excellence.
Your Role
As one of our Care Managers, you will:
Oversee the care operations for clients receiving Home Care Packages (HCP) and private services.
Ensure client assessments, care plans, and service delivery are not only clinically sound but also aligned with regulatory requirements under the new Aged Care reforms.
Act as a care leader and key contact for both internal staff and external stakeholders including GPs, allied health, and families.
Support governance and continuous improvement by assisting with internal audits, incident management, care documentation reviews, and quality initiatives.
Mentor support workers and case managers in clinical understanding, escalation protocols, and best-practice service delivery in the home.
Key Responsibilities
Conduct and review in-home assessments and develop tailored care plans in consultation with clients and families.
Facilitate all necessary care to clients.
Ensure compliance with the Aged Care Quality Standards and support alignment with the new Aged Care Act reforms coming into effect.
Maintain accurate, timely clinical documentation and oversee risk and incident reporting.
Liaise with funding bodies and regulatory agencies as needed.
Work collaboratively with the leadership team to improve systems, governance, and service delivery models.
Provide education, mentoring and support to frontline staff and new starters.
What You Bring
Preferred allied health background preferred (Registered Nurse (AHPRA) / Physiotherapist / Allied Health Professional) but not necessary with a minimum of 3 years' experience, ideally in aged care or community health.
Previous experience in a care manager, clinical coordinator, or team leader role within home care is highly regarded.
Strong understanding of Home Care Packages and associated compliance requirements.
Minimum 1+ year management experience with Home Care Packages is preferred.
Working knowledge of the new Aged Care Act and upcoming changes to regulation and governance.
Excellent communication, leadership, and time management skills.
Compassionate, adaptable, and confident, working autonomously in the field.
Current driver's licence and reliable vehicle.
Australian work rights are required for this position.
