About the Position
As a Community Services Coordinator, you will support our customers to live fulfilling lives while overseeing the day-to-day operations of one of our service sites in the Perth Metro Area.
We currently have opportunities across North, Central East/West, and South Metropolitan Regions, with a mix of permanent full-time and fixed-term 12-month contracts.
Key Responsibilities
- Facilitate and maintain high-quality Customer Support Plans and associated documentations.
- Supervise Support Workers, providing mentoring and development; manage daily activities including leave requests and contract changes.
- Oversee customers' NDIS pre-planning, goal progress, reporting, and plan utilisation.
- Support professional development of Support Workers through supervision meetings, Ability Check-In process, and performance management if required.
- Manage operating budgets and meet relevant KPI's.
- Efficiently manage rosters and customer funding packages.
- Ensure timely updates to document and customer information, including CMS data integrity.
- Report, manage, and monitor incidents, investigations, and complaints with a focus on prevention and continuous improvement.
- Participate in Quality & Safeguarding Commission activities.

About You
To begin our journey with us, you will ideally possess:
- Demonstrated supervisory experience in a related role (preferably in the Disability Sector)
- A tertiary qualification (or equivalent) in Health, Disability, Social Services, Behavioural or Sciences is preferred.
- Strong understanding or NDIS principles and the Quality & Safeguarding Framework.
- Excellent communication, interpersonal, and organisational skills.
- National Police Clearance (issued within the last 6 months)
- Current Driver's Licence
- NDIS Worker Screening Check & NDIS Worker Orientation Module
- First Aid Certificate (HLTAID010 minimum)
- COVID-19 Vaccination Certificate
- Full Australian Working Rights.
