Support Agency provides Support Coordination and Allied Health Supports to participants facing an array of complexities, working collaboratively as a team, we are able to offer our participants holistic support to get the best outcomes for them. We are proudly person-centred and achieve goals by establishing the right supports to build participants and their stakeholders' capacity. We work flexibly and creatively for each individual.

Qualifications & experience

Diploma or Degree in Community Services, Allied Health or related

Experience as a support coordinator, case manager or similar

Ability to work autonomously and flexibly

Kindness, compassion and interpersonal skills

Fantastic time management, communication and problem solving skills

Experience working with participants with complex psychosocial disabilities

Car and Australian drivers licence is required

Tasks & responsibilities

Implement, budget and monitor NDIS plans

Gather evidence and write strong supporting reports

Work with an array of people of varying abilities and complexities

Work in consultation with stakeholders
