Clinical decision-making responsibilities
Subacute inpatient aged care (GEM/Rehab), multi-site

We’re seeking an Occupational Therapist – Grade 2 (Aged Care) for our client to join a subacute inpatient team across multiple sites in Melbourne’s west. You’ll deliver evidence-based, person-centred OT to older adults, drive safe and timely discharges, and support junior OT/AHA staff within a collaborative multidisciplinary team.

What you’ll do

Complete functional & brief cognitive assessments, set occupation-focused goals, and deliver interventions that maximise safety, independence and participation.

Lead discharge planning (including home visits when required), equipment prescription and home modifications, coordinating with families, carers and community services.

Contribute to ward flow/throughput (GEM/Rehab), document clearly in the EMR, and participate in MDT meetings and ward rounds.

Supervise/coach Grade 1 OTs and AHAs; provide feedback, in-services and case reviews.

Participate in quality improvement and research aligned to best-care standards.

What you bring

AHPRA registration as an Occupational Therapist.

Consolidated Grade 2 capability in subacute/aged care: clinical reasoning, prioritisation, discharge planning, and interprofessional practice.

Strong communication and collaboration skills; comfortable supervising juniors and contributing to education.

Experience in quality improvement/research and the use of appropriate assessment tools.

Salary & benefits

Base (OT Grade 2): typically $90,235 – $103,724 (experience-dependent)
