Our client is a growing NDIS provider committed to person-centred support, dignity, and client choice. They are driven by purpose, not box-ticking and are looking for a Service Coordinator who can build relationships, guide support workers, and ensure quality, compliant service delivery.

About the Role

Working closely with participants, families, and frontline staff, you’ll act as the key link that keeps supports running smoothly. This is a hands-on, community-focused role involving home visits, safeguarding responsibilities, service planning, and staff guidance.

Reporting to the Service Manager, you will oversee day-to-day coordination, support quality, and ensure documentation meets NDIS standards, all while helping clients feel heard, respected, and empowered.

This role is part-time (4 days per week) with some travel across the Perth metro area.

What We Need From You

Support participants and families as the primary point of contact

Conduct home visits, risk assessments, and service reviews

Onboard new participants and complete NDIS-required documentation

Monitor support worker compliance and quality of service

Oversee rosters and assist with shift coverage where needed

Escalate risks, incidents, or safeguarding concerns appropriately

What You’ll Bring

Experience in disability, community services, or a related field

Knowledge of NDIS standards, Code of Conduct, and compliance expectations

Strong relationship-building and communication skills

Ability to work independently and organise competing priorities

Confidence in documentation, reporting, and incident management

Current driver’s licence, NDIS Worker Screening, NPC, and First Aid/CPR
