The Derbarl Yerrigan Health Service (DYHS) was established in 1974 and is the largest Aboriginal community-controlled health service provider in Western Australia, providing care to over 20,000 Aboriginal patients across four clinics in East Perth, Mirrabooka, Maddington and Midland. Our staff enable the delivery of culturally responsive primary health services, focusing on prevention, early intervention, comprehensive care and care coordination including chronic disease management.

About the Position

The primary responsibility of this role is to provide Primary Health Care for Aboriginal individuals, families and community groups. Aboriginal Health Practitioner within Derbarl Yerrigan Health Service assists in arranging and coordinating health care plans for clients. The essential criterion of this role is that the successful applicant must be an Aboriginal and/or Torres Strait Islander. This will be a Maximum Term contract for a period of 1 year from start date (with possibility of extension / permanency).

Key Responsibilities

  • Use the Communicare Data base to check recalls required for your clients and carry out data entry as required.
  • Work in partnership with clients and address the needs and reasonable expectations of each client.
  • Provide screening and any other procedures indicated and as requested by the GPs.
  • Perform the required tests to conduct a chronic disease care plan.
  • Encourage clients with chronic disease to participate in chronic disease care planning.
  • Provide support and coordinate a suitable management plan.
  • Ensure client accessibility to other organisations and advocate on behalf of clients.
  • Provide disease prevention, independent living skills and respite availability education to clients and carers.
  • Assisting families to access health and social services provided within the community.
  • Ability to recognise own training and special development needs.

Who we're looking for:

Essential

  • Aboriginal and/or Torres Strait Islander pursuant to Section 50D of the Equal Opportunity Act.
  • Current Registration with the Australian Health Practitioner Regulation Agency as an Aboriginal Health Practitioner.
  • Certificate IV or Diploma in Aboriginal Health
  • Knowledge of current health issues, impacting on Aboriginal Health.
  • Understanding the principles of public health, including health education, health promotions and disease prevention.
  • Well-developed written and verbal communication skills.
  • Knowledge and understanding of the holistic approach to health service provision.
  • Experience working in a multi-disciplinary team environment.
  • Demonstrated ability to maintain confidentiality and security of records and information.
  • Knowledge of Occupational Health & Safety, Industrial Relations and Equal Employment Opportunity Legislation.
  • Current Senior First Aid Certificate
  • Computer literacy skills

Appointment Pre-requisites

  • Successful Criminal Record Screening Clearance
  • Successful Pre-Employment Health Assessment
  • Successful Working with Children Check
  • Current WA Driver's license

About Derbarl Yerrigan Health Service

Princess Margaret Hospital held monthly clinics at the Aboriginal Advancement Council to provide immunisations and paediatric medical treatment. Princess Margaret Hospital supported the service as they recognised that our young Koolunga needed more follow up care. The monthly clinics increased from monthly to fortnightly and then weekly. Doctors and nurses from Princess Margaret Hospital also visited Allawah Grove Noongar Camp on the fringes of the metro area.

From this initial activity the seed was thus planted recognising that more health and medical support was desperately needed for all Aboriginal people not just the young. Princess Margaret Hospital should be recognised and acknowledged for the services they provided to Aboriginal families in the late 60's - early 70s.

The seed that was planted grew within the Noongar community and as a priority the next phase of the journey began. The word spread to the Noongar people and they formed the New Era Aboriginal Fellowship (NEAF) Committee - this Committee was inspirational in lobbying for contributions and community support to allow an independent, dedicated facility to be established where extended health care could be provided to those in need.
