Employment and Case Coordinator

Location: Cockburn, WA
Hours: 4–5 days per week (some flexibility available)

We are seeking an Employment and Case Management Coordinator to join our supportive, multidisciplinary team in Cockburn. This role suits someone with a background in health or behavioural sciences who thrives on coordinating services, empowering clients, and facilitating recovery and return-to-work outcomes.

About the Role
In this role, you will:
Provide case management support to clients with health conditions, injuries, or disabilities as they navigate recovery and vocational goals.
Coordinate communication between clients, employers, allied health professionals, insurers, and referrers.
Assist with employment readiness activities such as work trials, resume development, and job-search support.
Monitor progress and outcomes through structured case plans and regular reviews.
Maintain accurate case records, reports, and billing documentation.

About You
Minimum bachelor’s degree in Health Science, Nursing, Behavioural Science, Rehabilitation Counselling, Occupational Therapy, Psychology, or similar.
Strong interpersonal, organisational, and written communication skills.
Experience (or keen interest) in vocational rehabilitation, return-to-work coordination, or allied health environments.
Demonstrates strong attention to detail and a tenacious, solution-focused approach
Compassionate, professional, and adaptable approach to working with individuals with diverse needs, including those who are neurodivergent.
