About the Role
Are you passionate about empowering individuals to achieve their goals and navigate the NDIS with confidence? We are seeking an experienced and dedicated NDIS Support Coordinator to join our team on a 3 month fixed-term basis to cover long service leave. This role is critical as we transition into a new operating model, and we are looking for someone who can start immediately.
Working closely with participants, families, and service providers, the Support Coordinator will assist individuals to understand and implement their NDIS plans, build capacity, and connect with appropriate supports.
Key Responsibilities
Assist participants to understand and implement their NDIS plans.
Coordinate a mix of formal, informal, mainstream, and community supports.
Support participants to develop service agreements and create service bookings.
Monitor plan budgets and ensure services are delivered within funding parameters.
Build participants’ skills and confidence to manage their supports independently.
Advocate on behalf of participants regarding housing, income support, employment, health, and legal matters.
Conduct risk assessments and develop safeguarding strategies.
Promote participant rights, choice, and control in all aspects of service delivery.
Maintain effective relationships with mental health services, government agencies, and community organisations.
Maintain accurate records and prepare reports in line with NDIS requirements.
About you
We are looking for someone who has:
A minimum Certificate IV in Mental Health or equivalent
Minimum of 12 months experience as an NDIS Support Coordinator
Demonstrated experience in the mental health industry or community services
A strong understanding of the NDIS and psychosocial disability supports.
Experience in recovery-oriented, trauma-informed, and person-centred practice.
Excellent coordination, case management, and communication skills.
Ability to work independently and collaboratively, managing multiple priorities.
Proficiency in documentation and reporting aligned with NDIS requirements.
The successful applicant will also be required to provide a current National Police Check (or be willing to obtain), an NDIS Worker Screening and a current WA Driver’s Licence.
