About the Role

We’re recruiting for full-time Social Worker to help build out and manage our health programs, these include our Medical Fitness Programs, Veterans Health Programs and many others . In this role, you’ll work closely with a multidisciplinary team to deliver outstanding clinical care, help patients navigate life challenges and improve wellbeing whilst organising appointments for our patients to help foster a strong sense of community within the brand.

You will:

Be the glue between our clinical services

Provide case management and service navigation

Deliver assessments and tailored healthcare journeys for our patients

Build meaningful connections with patients to support long-term adherence and outcomes

Work collaboratively with exercise physiologists, physiotherapists, dietitians and other health professionals

Contribute to a culture focused on results, engagement, and community

What We’re Looking For:

Qualified Social Worker and registration with AASW

Accredited Mental Health Social Workers (AMHSW) through AASW

Confident with both case management and service navigation

Strong interpersonal and communication skills

A positive, professional attitude that adds value to both the team and member experience

Passion for working with the over-50s and promoting preventative health

Locations: Multiple Club Active sites across Australia, remote or at our Support Office on the Gold Coast
