The Role
As client Outcome Facilitator you will be responsible for the provision of case management, advocacy and support for women and children engaged through our Supported Housing Program.
Employed to provide holistic support for families and children throughout all facets of the client journey from intake to exit, an inherent requirement of this role is to provide case management, which focuses on client needs and fosters client capacity towards independence.
A key focus of this position is to assist in enabling clients who have been homeless or are at risk of homelessness to reconnect with their community by providing support and skills to sustain safe, affordable accommodation within the context of approved case management plans. The role encompasses working with families and children with complex needs to increase skills to sustain current and future tenancies and improve overall quality of life. The emphasis of the position is to assist in enabling clients to overcome barriers which have contributed to housing instability through capacity building to access safe, affordable, independent housing. Previous experience with Families and Youth is highly desirable.
This part time role has some flexibility in scheduling and coverage of hours.
This is a Permanent, Part Time Position (20 hours per week) role based in South Brisbane.
To be successful in the role, you will meet the following criteria:
- Minimum Certificate III in Community Services (Essential)
- Knowledge of and ability to work within relevant legislation and guidelines relating to the delivery of case management and safety and risk within the program area of operation
- Previous experience as a Support Worker or equivalent within the program area of operation
- Personal aptitude for working within set guidelines and adhering to all relevant policies and procedures
- Well-developed interpersonal, communication and organisation skills including the ability to exercise judgement and contribute to a team environment
- Ability to deliver advocacy on behalf of clients and ensure confidentiality of client information at all times
- Understanding of and commitment to the philosophy of the St Vincent de Paul Society Queensland and an ability to work in accordance with policies and procedures to promote the values and mission of the Society
