We are seeking a skilled and compassionate Clinical Care Coordinator (APS 6) to join a dynamic and supportive team. This role is designed for an experienced clinician to deliver high-quality, case management and support to individuals with complex mental health and psychosocial needs. As part of a multidisciplinary approach, you will work closely with clients and their families to navigate and access appropriate mental health care, helping to reduce barriers and improve engagement with services.

What You Will Do

In this role, you will:

  • Provide clinical case management to individuals with complex or multiple needs, using evidence-based interventions such as motivational interviewing, brief interventions, and psychoeducation.
  • Develop and maintain tailored care plans, ensuring that each client's mental health and psychosocial needs are addressed effectively.
  • Collaborate closely with internal and external stakeholders, such as healthcare providers and community services, to ensure seamless and integrated care for clients.
  • Take responsibility for maintaining accurate and confidential client records, ensuring all case notes, assessments, and treatment plans are up-to-date and aligned with best practice guidelines.
  • Foster a supportive environment that encourages client engagement, helping individuals navigate and access appropriate mental health services while respecting their autonomy and needs.
  • Participate in clinical supervision and ongoing professional development to enhance your expertise and maintain registration with relevant professional bodies.
  • Engage in case reviews, contributing to team discussions and ensuring that services remain responsive to client needs.
  • Support the development of group programs and community-based initiatives, contributing to broader mental health outreach efforts.

Skills & Experience

To be successful in this role, you will need:

  • Proven experience in delivering high-quality clinical services to vulnerable populations, with a particular focus on mental health and psychosocial support.
  • Strong interpersonal skills, with the ability to build rapport and work collaboratively with a range of stakeholders, including clients, family members, and external service providers.
  • Experience conducting thorough assessments and managing cases that require a sensitive, client-centred approach to care.
  • Excellent written and verbal communication skills, with the ability to document and report on cases clearly and concisely.
  • The ability to manage competing priorities while maintaining a high standard of care and professionalism.
  • High-level IT skills and experience using client management systems to record and track case details.

Mandatory Requirements

  • Australian Citizenship and the ability to pass a baseline security clearance.
  • Working with Vulnerable People registration or the ability to obtain one, or
  • Working with Children check or the ability to obtain one.

Additionally, candidates must hold one of the following qualifications:

  • Full AHPRA registration as a Psychologist, Occupational Therapist, or Registered Nurse, OR
  • Membership with the Australian Association of Social Workers (AASW), OR
  • A Master's-level counselling qualification with PACFA registration as a Registered Clinical Counsellor, or ACA Level 3 or Level 4 registration.
