Rural Health Tasmania are a proud Business Tasmania 2025 Employer of choice, we have an exciting opportunity for a Registered Nurse to join our team.
We are looking for a Registered Nurse with relevant post-graduate Mental Health Qualification or ability to achieve this with Rural Health Tasmania support. The role is part time in a community setting, with no weekend or shift work. Supporting clients with severe and complex mental health on the North West Coast of Tasmania.
Some of the key tasks and responsibilities;
1. Provide clinical assessment and triage with new clients using best-practice clinical assessment tools;
2. Engage and liaise with local GP practices, psychiatrists, psychologists, psychosocial and allied health services, as well as internal Rural Health Tasmania programs in line with the ‘Stepped Model of Care.’
3. Establishing a therapeutic relationship with clients and work within a team environment;
4. Liaising closely with family and carers as appropriate;
5. Input of data using the data systems provided by the organisation;

Qualifications and Experience
Must be a Registered Nurse with post-graduate qualification in Mental Health or the ability to undertake further studies to obtain relevant Mental Health qualification
Must have 2-3 years’ nursing experience in clinical or community settings; Care coordination and/or case management experience
Must be registered with APHRA
Must have a current WWVP registration Employment status
Must hold a current, unsuspended Drivers Licence
SKILLS REQUIREMENTS:
1.Demonstrated experience in working within a mental health setting and the ability to contribute to evidence-based practice in mental health; uses reflective practice to enhance work practice; identifies opportunities for improvement and works with management to implement improvements.
2. Proven knowledge, experience and understanding of Severe and Complex Mental Illnesses as outlined in the DSM5.
3. Sound knowledge of care coordination, referral pathways and Team Based Care principles, including care plan writing
4. Demonstrated knowledge of collaboration with allied health services, as well as Government and Non Government organisations
5. Write and articulate comprehensive and factual case notes as well as understand and use current assessment tools for the benefit of the clients
6. High level of written and oral skills, including the demonstrated ability to write and present clearly and concisely for specific targeted groups and prepare reports, papers and correspondence.
7. Sound understanding of the Social Determinants of Health
8. Evidence of essential communication, education, case management and communication skills, proficiency in communication technologies (computer: Microsoft Word, Outlook, Excel; email, mobile phone, etc.) and experience with health IT systems and reports
