The Role:
In this role you will provide a flexible, targeted Enhanced Maternal and Child Health (EMCH) service which maximises the health potential of identified children from birth to 6 years of age and their families, in particular where risk factors are known and families have high needs.
What does the role entail?
- Providing additional in-home support to families experiencing early parenting difficulties through family-focused and collaborative wrap-around care models.
- Providing high quality evidence-based practice with a focus on strengthening parenting capacity, improve family functioning and focus on the health and wellbeing of vulnerable children and their families.
- Empowering families to make optimal decisions related to their children's health and development through working with the protective primary carer.
- Health promotion through evidence-based education and information relating to all aspects of parenting including breast feeding, early learning, nutrition and immunisation.
- Early identification and intervention, improve linkages with other early childhood support systems including maternity services, family support, supported playgroup and early intervention services.
- Facilitating system processes related to intake, assessment, allocation and transfer of care for families on discharge to universal platforms where appropriate.
About you:
The successful applicant will have:
- Division 1 Registered Nurse (AHPRA)
- Registered Midwife (AHPRA)
- Graduate Diploma, (or Masters) in Maternal and Child Health, Family and Child Health, or equivalent qualification to meet MCH employment requirement in Victoria.
- Previous experience as an Enhanced Maternal and Child Health Nurse.
- Current Victorian driver's licence.
- Working with Children Check
The two days of work are flexible with a preference of Wednesday as one.
