About the Role:

The T2A Coordinator is responsible for overseeing and managing the delivery of transitional services, and the daily co-ordination of supported independent living programs to young people aged from 15-18 years. The T2A Coordinator will work collaboratively with General Manager – Youth & Communities, Senior Program Managers, Case Managers, Cultural Practice Advisors, Youth Workers, young people, their families, and external stakeholders to ensure that transitional services are delivered to the highest standard. The role will include the creation and implementation of individualised T2A care plans which encompass the transition to adulthood needs of the young person.

The T2A Coordinator is responsible for supporting the direct care operational division to support young people through life skill development, building supportive networks, and access essential services to achieve stability and independence as they transition from adolescence to adulthood. Responsibilities include overseeing and management of SILs programs, creating T2A plans, and ensuring young people are connected with education, employment, housing, and strong support networks.

About You:

You are an experienced and values-driven leader with a strong background in residential care, particularly in supporting young people from diverse cultural backgrounds through their transition to adulthood. You bring proven experience coordinating teams across SIL and community-based settings, with strong skills in case management, crisis response, and individualised support planning. With a solid understanding of legislative and compliance requirements, staff rostering under SCHADS, and team development, you are well-equipped to lead with confidence. You are highly organised, collaborative, and community-connected, committed to delivering safe, person-centred outcomes while fostering a supportive and high-performing team environment.

Requirements and Qualifications

Tertiary qualification in social work or a related field is desirable however a minimum Diploma of Community Services with significant sector experience, or related qualifications will be considered.

Minimum of 3 years of experience in managing residential care services or transition to adulthood services.

Knowledge of applicable legislation, regulations, and guidelines related to residential care and transition to adulthood.

Proficient in behavioural management techniques and crisis intervention.

Strong communication and interpersonal skills.

Ability to work effectively with diverse groups of people.

Attention to detail and accuracy in record-keeping and documentation.

Ability to manage multiple tasks and priorities in a fast-paced environment.

Certifications in First Aid, CPR, and Therapeutic Crisis Intervention.

A current Working With Children Check (Blue Card), current LCS Check and National Police Clearance, or willingness to obtain them.
