Community Coordinator (Clinical) - Aged Care

This position is responsible for successfully directing and coordinating all activities associated with care management including the care of potential and existing clients. You will provide leadership and effective management of the Care Management Team, compliance and clinical governance, and overall client experience including the delivery of quality client care services. The majority of clients supported by the Care Management Team are funded through the Home Care Package Program.

The Requirements

• Community or Home care experience in aged care is essential
• CHSP and HCP experience is essential
• Experience leading teams and supervising team members.
• Experience with strategic planning, and stakeholder engagement
• Practical experience adopting the Aged Care Quality Standards and experience participating in Quality Audits.
• Highly efficient and organised with proven time management skills.
• Strong engagement, influencing and negotiating skills which can be applied in a relationship management and sales arena.
• An effective communicator with excellent customer service skills and demonstrated problem solving abilities.
• Must hold a valid Australian Driver’s License
• National Police Clearance
• Current WA Driver’s License
• Fully Vaccinated
• NDIS Screening Check
