Position Description
Working across the continuum of community, the Acute Ward and Residential Care, our Social Workers support consumers and their families through their health and wellbeing journey. Within this role, the Social Worker provides high-quality, person-centred services to people experiencing psychological distress, people at risk of or experiencing homelessness, inpatients requiring psycho- social or welfare support and people requiring support and advocacy to access and navigate welfare and wellbeing services.
Responsibilities
The social worker will:
Consumer Care
● Provide comprehensive, accountable and high-quality Social Work care in accordance with the relevant legislation, professional and ethical frameworks, our Code of Conduct, and values.
● Work collaboratively with consumers, carers and other YDHS staff.
● Assess, plan, and support the implementation of appropriate interventions to help people live their best lives - whether in the community or in residential care.
● Initiate and actively participate in client / patient reviews.
● Support inpatient discharge planning including facilitating family meetings and monitoring post-discharge community integration.
● As a Family Violence Contact Officer, provide initial family violence support and information to YDHS staff.
● Undertake all mandatory and relevant training.
● Ensure a safe workplace for each other and our consumers.
● Enthusiastically participate in all capacity development and quality improvement activities.
● Delegate to, instruct and monitor Allied Health Assistants in relation to Social Work time and tasks in line with the Department of Health’s AHA Framework.
· Provide therapeutic counselling, applying evidence-based bio-psycho-social theories and strategies;
· Build strategic partnerships with relevant stakeholders including local services and organisations and regional mental health and family violence prevention networks.
Administration
● Accurate and timely collection of client data and service delivery statistics.
● Maintain medical records in accordance with the requirements of the law, the Department of Human Services and Health Service Policies.
● Uphold the client's rights to privacy, confidentiality, autonomy, dignity and safety at all times.
● Work within the scope of your role and escalate concerns.
● Ensure provision of a high-quality service by adhering to all relevant legislation, policies, protocols and standards.
Person Centered Care
● Ensure that all consumers, their families, visitors, and our colleagues are treated with respect and dignity.
● Listen to and collaborate with our consumers to understand their personal experiences to provide flexible care that complements their unique characteristics and supports them to live their best life.
● Support our consumers to understand and learn about their wellbeing and health.
● Involve consumers and their advocates in their lifestyle and healthcare decisions.
● Clearly explain your treatment options using easy to understand language and avoiding jargon.
● Respect lifestyle and healthcare decisions made by consumers.
● Ensure your practice and the service provided to consumers is free from discrimination based on age, gender, gender identity, sexual orientation, disability, employment status, cultural background, or religious beliefs.
Grade 2 social worker
In addition to the above:
· Provide supervision to social work students undertaking clinical placement at YDHS;
· Provide supervision to Grade 1 social workers;
Key Selection Criteria
Mandatory:
● Hold a Masters/Bachelor’s degree in Social Work and be eligible for membership to the Australian Association of Social Workers.
● Demonstrate understanding of Social Work theory and the ability to relate it to practice.
● Ability to work constructively and co-operatively within a multidisciplinary team and with a wide range of external service providers.
● Display efficient organisation, time management and prioritisation skills.
● Commitment to the YDHS values of care, compassion, respect, integrity, professionalism and safety.
● Professional and effective behaviour, interpersonal and communication skills including computer literacy
● Victorian Driver’s Licence.
Desirable:
● Experience in healthcare sector.
Relevant information
YDHS is responsible for employing and contracting to employees who meet stringent community and public sector expectations. Applicants shall be required to provide and maintain:
● An original National Police Record Check completed within the last twelve (12) months prior to commencement.
● An employee Working with Children Check.
● NDIS Check
● Relevant professional registrations.
● Complete a Pre-existing injury declaration form.
● Undergo any other relevant checks, education or licencing as directed at own cost.
