As a Community Disability Support Worker, you’ll be right there beside the people we support, helping them build independence, grow confidence, and achieve the goals that matter most to them. Some days you may be shooting hoops with the person you support, dancing to their favourite playlist, swimming at the beach or exploring a museum. Other days you could be supporting that person to navigate their day-to-day routines with dignity and independence.
At Rise, we provide 2:1 Community Supports (two Support Workers with one client), both at home and out in the community, depending on individual needs each day. This is your chance to make a meaningful impact and contribute to someone’s quality of life in a truly rewarding way.
We currently have vacancies working with clients based in Canning Vale, Como, and Rivervale.
What you’ll do
Empower the people you support by tailoring activities that build skills, foster independence, and help them reach their full potential.
Support daily living tasks including personal care as outlined in individual support plans.
Support clients to explore interests and hobbies to enhance social and community participation.
Build independence by encouraging decision-making and problem-solving.
Provide person-centred support aligned with individual NDIS goals.
Transport clients to activities, appointments, or adventures (using your own vehicle and Rise’s HiAce Van).

Essentials
A certificate in Human Services such as Disability, Aged Care, or Community Services.
Hands-on work with people with disabilities - bonus if you’ve supported clients with ASD, mental health, or intensive & complex positive behaviour support needs.
A commitment to learning and growth, with curiosity, self-reflection, and a team-focused approach
Availability to commit 3 days a week (Monday to Friday) 8.30am–4.30pm
Current driver’s licence with reliable own vehicle and willingness to travel across the Perth Metro area depending on client needs.
Comfortable driving larger vehicles, such as a HiAce van.
Desirable
Skilled in developing, delivering, and assessing plans that help clients achieve their goals
Experience implementing Positive Behaviour Support (PBS) strategies, reporting on Restrictive Practices and Familiarity with active support
Experience and/or training in de-escalation, trauma-informed practice, or MAYBO
✅ Formalities (The serious stuff!)
Appointments to this position require a NDIS worker screening check. Costs for new checks will be covered and/or reimbursed by Rise.
