My Care Pilot is seeking a passionate and experienced Support Coordinator to join our team. This is a rewarding opportunity to work independently while being supported by a reputable and ethical NDIS-registered organisation. You will assist NDIS participants in navigating their plans and achieving their goals, while enjoying the flexibility of managing your own schedule.

About the Role:

As a Support Coordinator with My Care Pilot, you will:

· Work directly with NDIS participants to implement their NDIS plans

· Help build participants’ capacity to navigate the NDIS and make informed choices

· Coordinate and connect participants with appropriate supports and services

· Maintain accurate and timely documentation and progress reporting

· Collaborate with stakeholders, service providers, and support networks

· Manage your own caseload with autonomy and professionalism (no set hours, no KPIs)

What We Offer:

· Hourly rate from $60-$75 per hour

· Work under My Care Pilot’s NDIS registration with full compliance support

· Ongoing guidance, support and training from our friendly team

About You:

We are looking for someone who is:

· Experienced in NDIS Support Coordination (Level 2 or Level 3)

· Knowledgeable of the NDIS and person-centred practice

· Organised, self-motivated, and able to work independently

· Compassionate, ethical, and committed to participant outcomes

· Holds a current ABN, Police Check, Working With Children Check

· Holds Professional Indemnity and Public Liability (PI/PL) insurance

· Qualification in Social Work, Psychology, Disability, or related field (desirable)
