Care FNQ is a registered NDIS service provider, and we’re excited to offer a fantastic opportunity for a passionate and enthusiastic Case Manager to become part of our supportive and close-knit team based in Cairns City.

About the Role

As a key point of contact for participants and support workers, our Case Managers play a vital role in what sets Care FNQ apart. You'll provide continuity, coordination, and a consistent presence for participants, their families, and other stakeholders – ensuring everyone feels supported and heard.

Key Responsibilities

Build and maintain strong, positive relationships with participants and their dedicated team of Care FNQ Support Workers

Assess individual needs and help tailor personalised support programs

Monitor and report on participant progress, and prepare documentation for NDIS plan reviews

Collaborate with support coordinators, house supervisors, and other stakeholders to ensure holistic care

Coordinate weekly staff rosters for participants, proactively managing any scheduling issues

Complete accurate case notes and regular client reviews, aligned with NDIS goals and plans

Follow all Care FNQ policies and procedures to maintain a safe, compliant workplace and ensure timely incident reporting

What We’re Looking For

Minimum of 2 years’ experience in the disability sector (highly desirable)

Solid understanding of the NDIS and the role of Disability Support Workers

Strong admin, communication, and report writing skills

Great time management and organisational abilities

A collaborative, people-first mindset with high emotional resilience

Competence in Microsoft Office and rostering systems

A current driver’s licence and your own reliable vehicle
