Aged Care Support (Community Services & Development)

Location: Newman, but regular travel to Tom Price and Marble Bar.

FULL TIME (Monday to Friday) doing 8am to 4pm

THE ROLE:

EPIS is seeking a Day Centre Team Leader to join the East Pilbara Independence Support (EPIS) team in Newman WA. We have an exceptional opportunity for a Team Leader to provide aged care support services to our community clients. We are currently looking for a full-time applicant for this role and can start immediately.

EPIS is a leading provider in quality Aged Care and Respite Services in the Inland Pilbara and Western Desert.

EPIS provides person-centred care by delivering services based upon understanding and assisting clients to live and achieve an appropriate degree of independence according to their own abilities, specific needs, and family and community support.

This position is responsible for the monitoring and delivery of care and services and promoting a culture of teamwork, effective communication and leadership to the Day Centre teams.

This is a hands on role and we are seeking applicants with aged care experience in a supervisory capacity.

This position will include travel to our satellite locations of Tom Price and Marble Bar.

Responsibilities of the role include:

Providing a range of community aged care services inclusive of personal care, domestic assistance, social support and transport

Provide support, assistance and supervision to EPIS Day Centre team members across Newman, Tom Price and Marble Bar.

Support clients to access their community, engage with activities of their choice, attend appointments and access amenities

Providing information and support to potential clients regarding aged care services available from EPIS.

Supporting and promoting the culture and values of EPIS evidenced through personal commitment, behaviour, language and the achievement of results.

What we are looking for:

Prior experience as a Team Leader or Coordinator.

Must have Cert III or Cert IV in aged care/individual support required.

Thorough understanding of contemporary aged care service delivery issues in rural and remote areas including Aboriginal communities

Knowledge and understanding of HCP and CHSP

Liaising with External Agencies

Ability to lead a small team of staff and promote teamwork.

Excellent communication and advanced relationship building skills

Full WA Driver's license

Current and valid Police Clearance
