About us:
Transition Care Program (TCP) provides short-term care that seeks to optimise functioning and independence of older people after a hospital stay. Transition Care Program is goal-orientated, time-limited and therapy focused. It provides older people with packages of services that include low intensity therapy, such as physiotherapy and occupational therapy as well as social work, nursing support or personal care. It seeks to enable older people to return home after a hospital stay rather than enter residential care prematurely.
About the role:
The Clinical Nurse demonstrates specialised nursing knowledge and skill in the assessment and support to access appropriate rehabilitation services post an acute hospital episode of persons 65 years & over living in the Central Queensland area. The position operates within a multidisciplinary team setting, promoting best practice and quality client care using a primary health care approach.
About you:
- Appointment to this position requires proof of qualification as a Registered Nurse with the Queensland Nursing Council and maintain the registration requirements with the Australian Health Practitioner Regulation Agency (AHPRA).
What we offer:
Total remuneration value of up to $131,900 p.a. is comprised of:
- An hourly rate between $54.44 - $58.30 p.h.
- 12.75% employer superannuation contribution
- 17.5% leave loading
