About the Role

We are seeking an experienced and passionate Care Manager (Clinical) to join our growing team. In this leadership role, you will oversee the clinical and care management of clients receiving services through the Support at Home program and Home Care Packages.

Key responsibilities include:

Conducting comprehensive assessments and developing person-centred care plans

Providing clinical oversight and guidance to carers and support staff

Coordinating services in line with client goals, preferences, and regulatory requirements

Building strong relationships with clients, families, and healthcare professionals

Ensuring compliance with Aged Care Quality Standards and organisational policies

To be successful, you will have:

Current AHPRA registration as an RN/EN or Allied Health

Strong clinical knowledge, ideally in aged care or community nursing

Experience with care coordination, assessments, and compliance requirements

Excellent communication, problem-solving, and leadership skills

A passion for supporting older adults to live safely and independently at home

Current driver’s licence
