The Role

We are seeking a passionate and committed Social Worker to join our team. As a Social Worker at ACNT, you will provide professional support to clients and their families, advocating for their needs, developing individualised plans, and connecting them with services and resources. You will collaborate with clients, families, support coordinators, and other allied health professionals to ensure holistic and integrated outcomes. This position is offered as a permanent part-time role, three days per week.

Key Responsibilities

Support clients emotionally and practically, linking them to key resources to help clients cope with life challenges.

Build strong relationships and networks with mental health teams to enhance client support and outcomes.

Advocate for clients to ensure their rights and needs are respected.

Facilitate access to community resources, services, and programs.

Work closely with multi-disciplinary teams to deliver coordinated, person-centred care.

Maintain accurate and confidential client records.

Uphold the NDIS Practice Standards, Assad Care Code of Conduct and the AASW Code of Ethics 2020.

Adhere to Assad Care organisational policies at all times.

What We Are looking For

Bachelor's degree in Social Work.

AASW membership.

Minimum 2 years’ experience working with individuals with disabilities.

Experience working with people with psychosocial needs and diagnosed mental health conditions.

Strong skills in advocacy, case management, and crisis intervention.

Excellent interpersonal, communication, and problem-solving skills.

Cultural competency and commitment to inclusive practice.

Mandatory Requirements

Current NDIS Worker Screening Check.

National Police Clearance (within last 12 months).

Working with Children Check (Ochre Card).

Current First Aid Certificate.

Completion of the NDIS Worker Orientation Module.
