The Role

We are looking for dedicated and compassionate Support Workers to join our team. In this role, you will provide supports to individuals in their homes and communities, helping them to live meaningful and independent lives.
You will assist clients with daily living tasks, community participation, and skill development, while promoting their rights, choices, and well-being.

As a key member of our team, you will work closely with clients, families, and other professionals to deliver holistic, high-quality care aligned with ACNT’s values and the NDIS Practice Standards. This position is offered as a permanent part-time role, three days per week.

Key Responsibilities

Support clients to engage in age-appropriate tasks and activities, including in-home support, community access, and employment.

Provide direct care as outlined in the individual support plan.

Assist clients in accessing social and recreational activities.

Monitor client well-being and promptly report concerns to the management

Maintain accurate documentation

Uphold professional conduct aligned with ACNT values and legislative requirements.

Adhere to Assad Care NT policies and procedures and the NDIS Practice Standards at all times.

What We are Looking For

Certificate III or IV in Individual Support, Disability, Aged Care, or equivalent.

Certificate in Mental Health or equivalent training.

Demonstrated experience supporting people with disabilities, including mental unwellness.

Excellent communication and interpersonal skills.

Ability to work independently.

Understanding of NDIS Practice Standards and Quality Indicators.

Computer literacy (email, online systems, mobile reporting tools).

Mandatory Requirements

Current NDIS Worker Screening Check.

National Police Clearance (within last 12 months).

Working with Children Check (Ochre Card).

Current First Aid Certificate.

Completion of the NDIS Worker Orientation Module.
