About the role

Community Care Hub Australia PTY LTD' is seeking dedicated Disability Support Workers for IMMEDIATE start to join our team in Greater Western Sydney Region. This important role will involve providing compassionate and personalised support to individuals with a range of disabilities, helping them to live independently and participate fully in their communities.

Disabilities range from ADHD, ASD Level 1-2-3, ABI, GDD, MS, Down Syndrome and more.

What you'll be doing

Assisting clients with daily living activities such as personal care, meal preparation, and household tasks

Accompanying clients to appointments, social activities, and community engagements

Advocating for clients' needs and ensuring their rights are respected

Documenting client progress and maintaining detailed case notes

Provide feedback to the Service Coordinators and other stakeholders

Collaborating with allied health professionals and other support services to develop and implement individualised care plans

Adhering to all relevant policies, procedures, and safety guidelines

What we're looking for

Relevant qualifications in Disability Support (Certificate III or above), or Community Services/related field

Experience providing person-centred care and support to individuals with a range of disabilities

Strong communication and interpersonal skills, with the ability to build trust and rapport with clients

Patience, empathy, and a genuine commitment to promoting client independence and wellbeing

Problem-solving skills and the ability to think creatively to overcome challenges

National Police Check

First Aid + CPR

Working With Childrens Check

NDIS Screening Check

NDIS Worker Orientation Module “Quality, Safety and You” Certificate

A current driver's licence and access to reliable transport
