Right at Home Sydney Parramatta is seeking qualified, reliable and passionate Registered Nurse (Care Coordinator/Community Health). The successful candidate will coordinate, manage, and administer the delivery of care services to our clients.
Purpose of Position: This role is customer focused and involves consulting with families, preparing in home care assessments, and drafting care plans to best meet client requirements.
Duties include:
Meeting with clients and assessing their needs. This role will handle anything from information requests, feedback, general enquiries to complaints.
Preparing in-home care assessments and drafting care plans to best meet client requirements.
Recruiting, training, and managing the performance of the care team.
Maintaining client and care worker compliance and documentation according to government standards.
Ensure all documentation is accurately and comprehensively completed in Right at Home databases and software programs in accordance with organisation policies and procedures and legislative frameworks.
Provide direct nursing care and/or individual case management to aged people and People with disabilities.
Provide health education to improve the health outcomes for individual People or groups
Communicate effectively with the NDIS Program Manager in relation to program issues or opportunities for improvement
Attend and contribute to care team meetings, supervision meetings, and other relevant meetings as required and use information/feedback to continuously improve work performance
Assist in the delivery of person-centred service according to the objectives of each program
Identify early preventative actions for health concerns that may impact on other associated issues such as behaviour
Liaise, plan and coordinate health related services with key stakeholders in providing a high quality care based on individual need
Assess, review, develop and educate staff on individual health care plans and procedures.
Support staff in medication administration, and support quality improvement processes
Essential Criteria:
Current AHPRA registration and Australian Working Rights
Bachelor’s degree with previous experience of 1 years full time or equivalent part time in a fast-paced environment
Permanent Residents or citizens of Australia will be preferred
Current Australian National Police check, Working with Children Check, and Current First Aid & CPR certificate
Excellent verbal and written communication skills

Updates knowledge and skills by pursuing continuing professional development
Time management and organisational skills
Ability to manage, direct and supervise staff, coordinating and assessing work activities
Nursing skills including wound care and medication management
Knowledge of high prevalent risks, risk assessments, risk mitigation
The ability to operate efficiently and deliver excellent service when under pressure
Time management, organisational skills & ability to manage competing priorities
Relationship development skills to support diverse clients to achieve their goals and objectives
Intermediate to Advanced Microsoft Office experience (Excel, Word, Outlook, and CMS)
Job Type: Full-time
Pay: AU$76,000.00 – AU$86,000.00 per year
