St John of God Midland Public and Private Hospitals are co-located in the same facility, and opened in 2015. The hospitals provide both public and private services for people living in Perth's eastern suburbs and nearby regional areas. The hospital includes an Accident and Emergency Department, critical care unit, mental health unit, and maternity ward.
Your role
We have an exciting opportunity for a compassionate and skilled Social Worker to join our general medicine team on a full-time, fixed-term basis (up to 12 months). Working as part of a supportive and collaborative multidisciplinary team, you will deliver high-quality clinical social work services to patients, drawing on evidence-based practice and a patient-centred approach.
You will play a key role in supporting patients and their families through complex health journeys, ensuring care is aligned with the Australian Association of Social Workers Code of Ethics, Practice Guidelines, and SJGHC policies and procedures.

The Position
- Demonstrated clinical knowledge and skills in assessment, planning, implementing and evaluating Social Work interventions.
- Provide Social work services to DVA, TAC and Workcover patients.
- Actively intervene with appropriate counselling, support, education, community liaison and referrals.
- Actively participate and where appropriate, lead family meetings.
- Participate in multi-disciplinary service approach for clients, including contributing to the evaluation of the Social Work service and Clinical Specialty.
- Deliver patient-centred care, building health literacy and promoting self-management.
- Demonstrate reflective practice and commitment to professional development.
- Supports Social Work service by communicating issues and ideas for enhancing the service.
- Supports the profession through Social Work student learning.
- Liaise with compensable bodies, financial institutions and external care agencies where appropriate.
Position Requirements
To be considered for this role, you will hold a relevant tertiary qualification in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW). You will bring demonstrated skills in social work assessment, case management, and discharge planning, with a strong commitment to delivering patient-centred care in a healthcare setting.
You will also have well-developed communication skills both written and verbal alongside the interpersonal capability to engage effectively with patients, families, and colleagues. Your ability to work collaboratively as part of a multidisciplinary team will be essential, as will your professionalism, empathy, and dedication to achieving positive outcomes for patients.

- Salary: $85,508 to $120,255 per annum, plus 12% superannuation
