The Opportunity
As a Team Leader within the Supported Independent Living (SIL) program, you will use your experience in client-centred case management to deliver quality services to Youturn clients, ensuring the needs and wishes of the clients are at the forefront of service delivery.
You will work directly with clients that are experiencing multiple and/or complex needs in the care of the child protection system, to deliver supports to increase life skill development in preparation to transition from care.
Key responsibilities of this role include:
Complete primary operational tasks and manage the client's journey through the service with a client centred approach.
Coordinate case management and regularly review case plans through liaison and collaboration with the Program Coordinator and/or Team Leader.
Provide information and assistance in accessing appropriate services and resources for support in accordance with documented procedures.
Develop and implement ongoing planning to support the clients needs, to effectively transition to, or maintain a safe, caring and supported living environment.
Ensure service delivery is coordinated through an integrated model within Youturn as well as external agencies (government and non-government).
Ensure culturally competent practice when working with Aboriginal and Torres Strait Islander families and linking with the Cultural Support Worker and appropriate stakeholders.
Maintain key program deliverables as outlined in individual work plans and job descriptions, and in line with the Human Services Quality Framework standards.
Essential Requirements
Tertiary qualifications and/or a minimum of a Cert IV qualification in the Community Services industry along with relevant industry experience.
Clear understanding of the current and relevant legislation, which includes the Child Protection Act.
Knowledge and understanding in the delivery of client centred case management and demonstrated ability in applying such principles.
Knowledge and understanding of complex needs of clients who are in out-of-home care who have experienced trauma and are facing severe barriers to achieving stability and security in their lives.
Demonstrated ability to build effective relationships with clients, which will involve having highly developed interpersonal skills and strong professional boundaries, which are an important aspect of working with complex young people.
An ability to interpret and understand Government legislation, specifically an understanding of relevant legislation, policies and practices that impact on service development and delivery. This includes knowledge of the Child Safety Service Standards and licensing requirements.
Demonstrated competence in working with people, including those from diverse cultural backgrounds.
Highly developed interpersonal and negotiation skills; including a proven ability to develop rapport and communicate with employers and staff from Government Departments, community agencies and training and industry organisations.
Knowledge of, or the ability to acquire, local community support services and networks.
Experience of behaviour and crisis management strategies.
Highly developed written communication skills in professional report writing.
Other Requirements
Successful applicants will also need to hold a ‘C’ class Open driver licence, State Working with Children Check and hold, or be eligible to apply for, a Licensed Care Suitability Check issued by the QLD Department of Child Safety, Youth Justice and Multicultural Affairs.
