Ahanacare is looking for an experienced and exceptional Welfare Support Coordinator/ Intake officer on a FULL-TIME basis to join our team at Morley, Western Australia, reporting to the Service Manager or Director.

Ahanacare is a registered NDIS Provider to deliver the following supports:
· Development-Life Skills
· Daily Tasks/ Shared Living
· Assist-Life Stage – Transition
· Personal Activities
· Community Participation
· Support Coordination (including Specialist)
· NDIS Recovery Coach
Role and Responsibility
• Liaise with all members of the support/care team and provide coordinated care.
• Carry out comprehensive and accurate client assessments.
• Contribute to the development, implementation, monitoring and review of care plans to meet client’s care needs and to promote quality of life.
• Promote a consultative and collaborative approach to client care planning and quality-managed activities with external stakeholders such as welfare and community service agencies, private entities, and government bodies (including Service Australia and NDIS)
• Monitor and report on the client’s progress.
• Maintain appropriate records and documentation.
• Support and assist clients in life skills development, mental well-being, and reaching individual goals in the community.
• Refer clients to appropriate agencies for additional help if necessary.
• Assist with the orientation of new staff and on going professional development of staff.
Essential Skills and Qualifications
• Current First Aid certification
• Relevant advanced diploma or higher qualification in community service or community development
• Member of ACWA is desirable
• Demonstrated knowledge and skills in assessment, treatment and evaluation
• Demonstrated ability to work effectively in a multidisciplinary team setting as well as autonomously
• Ability to work with people from diverse backgrounds with compassion and empathy and without judgment for their lifestyle choices, personal preferences and values
• Demonstrated experience and knowledge of NDIS and working within a legislative framework
Desirable
• A can-do attitude, enthusiasm and great communication skills
• Strong, proven interpersonal skills and empathy will be found in the right candidate
• Excellent prioritisation and organisational skills
• Proficient in a 2nd language other than English
