The Care in the Right Setting team at the Mackay Base Hosptial is seeking applications for a Nurse Practitioner to join their team on a permanent basis.

Your opportunity

The Nurse Practitioner demonstrates advanced and expanded clinical skills, knowledge and high-level capacity to translate these into contemporary evidenced based care for acute patients in Hospital in The Home or in a residential aged care environment. The Nurse Practitioner provides effective autonomous and accountable health care that incorporates application of high-level knowledge and skills at an advanced level of nursing practice, including assessment and management of clients to ensure safe and optimal care. This includes but is not limited to; the direct referral of clients to other health care professionals; prescribing medications; and ordering diagnostic investigations.

About the Team

The Care in the Right Setting program aims to improve access to health care and reduce the demand on hospitals by improving flow and promoting the right care in the right time for the right outcome. The Care in the Right Setting Team includes Hospital in The Home, Geriatric Evaluation Management/Rehabilitation – Hospital in The Home, Residential Aged Care Facility Support Service, Mackay QPS (Watchhouse) and Geriatric Emergency Department Intervention and four hospital funded beds at Glenella Care Residential Aged Care Facility awaiting permanent placement. Patients receive quality care that is tailored to their individual health care needs.
