About the role

Fixed term temporary part time opportunity to provide nursing care at an advanced scope of clinical practice including expanded skills and knowledge in assessment, care planning and implementation, diagnoses, and evaluation of nursing care for a cohort of consumer/patients in the specialty area of acute community mental health care for the Acute Care Team. This role leads the delivery of a Home-based Assertive Response Team (HART) model within TPCH Acute Care Team.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Provide specialist nursing interventions that demonstrate an expanded level of nursing knowledge and skills in the practice area of mental health
  • Demonstrate the ability to promote, undertake and support research activities to support improved client care and best practice.
  • Provide clinical leadership, mentorship and support for nurse practitioner candidates and other relevant staff.
  • Responsible for prompt and accurate entry and collection of consumer demographic information, clinical diagnosis, outcomes collections and service activity into relevant applications, including the electronic documentation of admissions, discharges, and transfers in compliance with legislative requirements under the Mental Health Act 2016.
